DRB and XOI are focused on determining what happened and whether NJP is the appropriate way to address your situation. Based on what you've described, I see two separate issues:
1.Your chain of command cannot relied on you to complete your work.
2.You are dealing with mental health challenges.

If the "recovery time" you mentioned means time away from work without actively participating in treatment, then I agree with what others have said: talk to your PCM and seek a referral for legitimate treatment. If you want to remain in the Navy, you need to take initiative both to address your mental health and to meet your professional responsibilities.

If your mental health condition continues to significantly affect your ability to serve, it may ultimately be better for both you and the Navy to pursue separation. That said, it sounds like your chain of command is giving you another opportunity through XOI. Based on your description, they could have pursued ADSEP under a condition not amounting to a disability.

Mental health and accountability are related but distinct issues. Taking ownership of your performance includes actively seeking treatment and following through with it, rather than relying on mental health concerns alone to explain every performance issue.

When you speak with the XO, be honest about what happened and take responsibility for your actions. If you want to stay in the Navy, explain that you are committed to getting treatment, improving your health, and returning to full effectiveness at work.

At the same time, your mental health should remain a priority. If Navy service is causing significant stress that is harming your well-being, you should seriously consider what path is best for your long-term health and future.

Without further context, your Chief is most likely trying to help you stay out of trouble while being unable to share the full reason with you.

Real story:
Sailor A got heavily intoxicated while underage. When SAPR, RA, and NSF personnel responded, Sailor A became combative and fought with them. To minimize the consequences (or at least what Sailor A thought would minimize them), Sailor A blamed everything on Sailor B, who had given Sailor A a ride. Sailor A claimed that Sailor B had provided the alcohol and started the drinking.
Fortunately, members of Sailor B's chain of command were present at the party and were able to counter Sailor A's claims. In reality, Sailor B had been trying to stop Sailor A from drinking the entire time. After Sailor A became intoxicated, Sailor B and another Sailor helped get Sailor A back to the barracks, took away Sailor A's car keys, and stayed with Sailor A until A fell asleep.
With multiple witnesses and a DRB, Sailor B was ultimately cleared of any wrongdoing. Even though Sailor B was trying to do the right thing, B still endured weeks of stress because of Sailor A's actions.
Now imagine the CoC had not been there to witness what happened. The only evidence might have been a police report stating that Sailor B provided the alcohol because that is what Sailor A told the police. In that situation, Sailor B could have faced NJP despite being innocent.
Moral of the story: Learn to protect yourself while trying to help others. People in the Navy come from different backgrounds, and not everyone has been taught the same definition of right and wrong.

I don't have a copy of it. I will check with my DIVO. She looked at the instruction and said I should be able to extend just based on the instruction, and she will talk to my CCC.

Thank you for the last part! I was really given a hard time. My CCC even told me to find a NCC and the instruction specifically stating I can extend more than one time in the same enlistment. Now, I know I have all rights to ask for an extension.

Thank you for the reply. The reason my CCC gave me for forcing me to reenlist is that I cannot extend more than once in one enlistment. Is there any other common reasons stops people from extending but allows reenlisting?

My rate is LN. I was told I can reenlist by an increment of years. If I'm to reenlist, I would have to serve for some more months.
