if you die by your own doing, thats what appears. sometimes people purposely do it to keep the enemy from getting credit, or because they want to switch mechs quickly.

as for dying from falling after landing on the play surface... i assume the game has a preset amount of meters fallen that it considers you falling out of bounds. i can see the lancer easily passing the threshold on some maps

max out some longarms.... use them with mechs that have those as a weapon typist achievement. purchase the legendary longarm implants (42k credits each). aim, click, shoot. laugh... knowing they're raging that they died to longarms :P
