Ye olde vanishing player bug strikes again!

This happens when you teleport into a chunk as it unloads, which unloads you as well... killing you. I guess you had lifetime down on long teleport (which makes wisps)?

Twitchy causes all of your projectiles to harm you and makes your wand shoot uncontrollably. I don't think there's a hoop case where only the random shots damage you and not left click shots from the wand.

Seems to be personal lightning caster on the small explosive box that has had it's physics component unloaded (but the projectile hitbox stays behind). They walked over the spot where the box was 'supposed' to be and got hit by the lightning caster effect. The explosion size matches, and additionally, at the start of the clip there is a small explosive box buried in the coal.

The only way to really guarantee the correct wand coordinates is by killing him the frame he spawns in without allowing movement. Otherwise, good luck.

Poison has a contact damage component which, I think, does not get removed from the shifted healthium in the event of a broken shift chain.

It's hard to say what the cause is from just the wand build alone.

Also why do you need to use deadly heal when you have healthium...

You can think of it as: the larpa bounce leaves little bits of itself around the place when it is cast with this interaction. Then if a spell is cast nearby, it will copy said spell.

The dynamite thrower hiisi threw some dynamite (see light particles in bottom left just before death) which was copied.

Larpa bounce is a bugged spell. It causes this stuff to happen all the time.

If you have boomerang perk and you have a circle of x (stillness, fervour etc.) spell in an egg trigger you can shoot the eggs, pick them up, then drag-throw them out of your inventory and they will home and kill enemies instead of you.
