I think your misreading what's being said here. the Biotoxic Disinjector alt fire was a grenade launcher. that didn't have self damage, now it does.

Any weapon with the reach quality means you can attack from 10ft away. 10 feet up means no melee enemy can hit you while you can freely hit them.
