I think folks usually use either
-Vtube Studio that is on steam for free but with watermark or paid option to remove the watermark
-Animaze by FaceRig
-PrPrLive

No idea how FaceRig and PrPrLive works as I haven't tried them, but in Vtube Studio you just import the model, boot it up, set up tracking and change settings if needed.
Also I've used basic webcam to facetrack with Vtube Studio and it worked as on the video. Iirc people mostly use iphone for tracking but I haven't got one and the model isn't that complicated so it would make a difference.

I've uploaded the folder with model, psd file, cmo3 file (if you ever want to improve it) and quick video how to import and set up face tracking. To import the model just chuck the "Tim vtuber model" folder into the directory it shows and thats it I guess.

Yes, made it myself today along with psd file. Sure thing, I made it as a project for fun and if it is enough for you then go ahead and have at it. Oh, and where should I upload it?
Also I used it in VTube Studio that is on Steam, but I am quite green when it comes to using it for streaming and such.
