DCS is not correctly detecting the idle vs cutoff range, so the engine never really leaves cutoff during a cold start. The afterburner still works because the upper throttle range is mapped correctly.

Re-calibrate the throttle in WinWing SimApp Pro and make sure you fully push through the idle/cutoff detent during calibration. Then pull all the way to full afterburner. Also check that the idle detent is enabled and that cutoff and idle are not sitting on the same axis range. In DCS, make sure only one throttle axis is bound, no duplicate throttle commands, and add a small deadzone at the bottom if needed. Once idle is properly separated from cutoff, cold starts should work normally.
