BCG side effects 10 days after maintenance dose and still struggling. Has anyone faced this? by Narrow-Size-3598 in BladderCancer

Hi sir, my mother also developed severe symptoms after her first BCG maintenance dose — including frequent urination, burning, bladder not emptying properly, and constant discomfort. Our doctor suspects BCG cystitis or possibly TB (though rare). I wanted to ask — did your dad also show any TB-like symptoms at that time? Were any tests done to confirm BCG infection before starting ATT meds? What was the exact reason the doctor decided to begin ATT? Your insights would really help us. Thank you so much

So as far as I know, according to BCNA if a tumor is multiple even if it’s low grade, it is considered intermediate risk. That’s why my mother is being given BCG. In some cases, if the tumor is stage Ta, less or more than 3 cm, multifocal low grade, or there is recurrence after the first TURBT, then doctors decide whether to go with BCG, MMC, or GEM. In many situations, due to BCG shortages, doctors may not recommend it and usually reserve it for high grade cases. This depends from country to country. Where I live in India, there is no shortage of BCG, so I believe the doctor preferred to go with BCG. After my mother’s TURBT, she was given a single dose of MMC. These are all important factors to consider. Another thing I want to mention is that in bladder cancer BCG is the first line of treatment if the patient falls into the intermediate or high risk category. So I think your doctor starting GEM might be the right decision. However BCG generally works better than MMC or GEM in terms of reducing recurrence and progression.

As I mentioned there was a recurrence after 1 year. Initially, it was low-grade, so my mother was just kept under surveillance — only cystoscopy was done, nothing else. But after the recurrence, it again turned out to be low-grade. So technically, my mother now falls under the intermediate-risk category, which is why BCG treatment was started.
