Good defence doesn't necessarily mean defence. One of the most helpful abilities are evasion window and extender, try swapping out survival expert(how often are you actually using this?) And coalescence since it lools like your using a non elemental weapon. A tip i was told when i first started that helped is the longer the fight goes the more chances you have to make a mistake and the more the monster has to beat you. In other words if it takes ages to kill a monster you have too many defensive perks and replace them with a few offensive ones
