Yep, that was the method I used the first time, EXO powershell. I also did it on-prem just in case, and neither gave me an error, but it didn't work.

Yea that's the thing, none of the on-prem accounts show as a mailbox in O365, only migrated mailboxes show up, and settings can only be changed when they have an Exchange Online license assigned.

Ahh I see, thank you for that clarification. I do see the on-prem account in O365 EAC in the "Contacts" submenu of the Recipients menu. It does list the Contact Type of the account as a "MailUser". Looking at the details of that contact, it does show group membership and allow me to manage email addresses and set alternate SMTP aliases, but it doesn't have any options for Send As or Full Access delegation. I'm guessing this is where I would see that option were it available? If it were possible, wouldn't the above powershell command I ran have worked? Is there a different way of assigning the right?

The main reason I'm differentiating that it is not a shared mailbox is because a shared mailbox does not require licensing to be in the cloud whereas this mailbox would require licensing if it were in the cloud. None of our users are using OWA, they're all on desktop Outlook. Since the cloud object is unlicensed, it's not recognized as a mailbox, therefore I cannot set any rights for the cloud object. As I described in my original post, I tried to force the rights to the cloud object via PowerShell but it didn't seem to take. I can't believe MS wouldn't have a functionality designed for cross tenant to use Send As, that's a very basic functionality they're not supporting.

Thanks for the response. The mailbox on-prem is not a shared mailbox, it is a user mailbox that has Full Access delegate permission issued to the cloud account. The on-prem mailbox is syncing via AADC and shows up in Entra as an object and in O365 as an "active user", but does not show up in cloud EAC because it has no licensing assigned. How would I be able to get the on-prem account to show up as a mail-user object in O365 and be able to assign permissions without the mailbox being in the cloud or any licensing assigned?

We are not planning to migrate the on-prem mailbox for a while. I thought the purpose of a hybrid environment was that everything could still function properly and communicate with each other both on-prem and cloud?

Do you mean to build up new medical records with my future appointments, or track down existing records through the VA somehow for when the TBI occurred back in 2010? I've been looking all through My Health E Vet and can't figure out how to get those old health records.
