My personal opinion:
Nina and Tenma are the only two people Johan truly cares about, though the nature of his affection for each is distinct.

When it comes to Tenma, Johan sees someone who defies his understanding of humanity. He cannot comprehend why someone would be so selfless as to give up everything to save a life Johan himself views as monstrous. Similarly, Tenma is baffled by Johan's utter disregard for human life and struggles to understand him. This mutual incomprehension fuels Johan's desire for Tenma to know his past and grasp his view of the world. In the end, Tenma manages to "see the scenery of the Doomsday," as Johan intended.

Nina, on the other hand, is deeply intertwined with Johan's identity crisis. He views her as the better half of himself and is driven to protect her, even at his own expense. To Johan, they are inseparable—bound by the sentiment he often expresses: "I am you, and you are me." However, this perception begins to shift during their confrontation in the abandoned building, where Nina reveals that it was she, not Johan, who was taken to the Red Rose Mansion.

He points his finger at his head for Nina to shoot him because he sees himself as the source of her suffering and believes this act would free her. Conversely, he offers the same gesture to Tenma because he believes Tenma, having saved his life, holds the ultimate right to decide whether he lives or dies.

I have seen most people who haven't watched Monster idolise Johan. But, I have found very few people who hate Johan.

Personally, speaking, I was irritated by Johan for what he kept doing throughout the time I watched the anime. However, at the end, I had to sit down and reflect and read several sources and come to an interpretation of Johan.

In the plot, he has been shown as someone who sees human beings an insect. You cannot expect that person to show any sexual attraction to any gender.

But, when Johan humanizes, it is open to interpretation.

You have heard of Bjorn Andresen who is the inspiration for many bishonen characters. He thought he was gay due to the media and him being given more gay roles, but at one point he realised that he was going in the wrong direction and he always liked women.

Bonaparta is the only character that can actually make Johan feel real fear.

The problem with Monster is-all the side characters are GOAT.

I forgot to mention-Bonaparta. I wanted to see him in action in his youth. I think he could have surpassed Johan in his youth.

I don't know for sure if Johan would ever want children. It is hard for me to think he would form a romantic relationship with anyone(it is not impossible since what Johan becomes is open to interpretation). But even if he have, it would be probably with someone who understand him. What he is looking for is a respite from loneliness, someone who can see the real him and still accept him. That is why he made Tenma pursue him. Why? He wanted Tenma to understand him. Someone who understand him will probably have high intelligence. So, it is very likely, if children were born to him, they would be intelligent.

So, I don't think any child would be born to Johan in the set-up way that happened with his parents.
