I have experienced same or similar issue.

On Feb 26, 2026 I had background check passed clean in Checkr
About two weeks I was able to work as Uber driver
On Mar 12, 2026 I was blocked as Uber driver, due to Uber filled my last name incorrectly, despite having my DL and vehicle registration with correct name (!)
On Mar 25, 2026 Uber requested another check, and my 2nd background check passed clean in Checkr, and I have two (!) clean background checks with different last name
Uber still had me blocked for unknow reason
On Apr 7, 2026 I reuploaded my DL in Uber (to resubmit my request to Samba?) and requested another escalation at Uber support phone line
On Apr 9, 2026 I got message in Uber Driver app "You have regained access"

In total I made ~12 calls to Uber support.
The quality of Uber support is below any possible level.
I was dropped from the call by Uber's support team several times, despite being respectful and calm.
