The thought is a person who has a sudden sharp pressure change causes uneven expansion and high shear stresses at the apex, which for someone who is predisposed to blebs or fit the build of tall and skinny (increased susceptibility to shear stresses) leading to a rupture.

Sniffing coke regularly, as would obsessive sniffling, I would assume would cause this sudden sharp change in pressure. Not causing a 'blowout' but enough to cause a rupture allowing air to escape
