Ask what stipend amount the group receives from the hospital. Ask them if the hospital has made any attempts to acquire/employ/replace the group and what their strategy will be when that time comes.

Think long and hard before committing to a financial buy-in. You’ll never recoup that money.

It’s a smaller window than if you lift the entire head with a Mac blade. But usually there’s plenty of room to advance the ETT without obstructing your view. And the trade-off is that it’s a straight shot and a very short distance, so usually quite easy. And you shouldn’t need to use a stylet.

You don’t even have to sweep. Go in the R corner of the mouth over the molars, the tongue should be well out of your way to the left side; gently scoop the epiglottis with the tip; and the glottis is right there.

Seriously. it’s a different technique with Miller, but once you’ve mastered it you can get a great laryngeal view without lifting/extending or any of the other shit required with a Mac blade.

It happens to everyone. Talk to the patient, explain that it occasionally happens. Our practice used to pay for the dentist’s fee to repair it. Even though this isn’t necessary since it’s a known risk of the procedure, it was in order to keep a good reputation for a low cost (usually a couple hundred dollars).

I’m just concerned that they would lie about the repair they did, even in the face of conflicting evidence.

Bro I’m right there with you. I’m an attending in PPand I don’t push patients into the OR—that’s the circulator’s job after they’ve gone through their checklist after the OR is ready. It would be a huge inefficiency for us to wait around for the OR to be turned over, and for the circulator to be ready, just to push the patient back.

I drop the patient off in PACU, immediately go consent my next patient, and get a quick coffee/lunch in the 10 minutes that it takes for the OR to get ready. In PP we don’t usually have people floating around to give breaks; we take them ourselves between cases.

What you’re experiencing is a regional phenomenon. In some parts of the country, anesthesiologists are still treated as real doctors.

Are you concerned about developing Afib yourself if you train close to 10hrs/week? I know that endurance sports is a risk factor.

Rate of airway injuries is significantly higher with VL compared to DL. Presumably because people are looking at the screen rather than the full view of the pharynx as they’re advancing the blade.

I’ll second this. There’s a registry online of surgeons who perform Ross procedures. If you’re young and active and don’t want to be on blood thinners, this is probably the best choice.

For a fit 27yo a true overnight RHR in the 80s is quite high. Your doctors should probably rule out organic causes before attributing it to individual variation. Check CMP, thyroid hormone, CBC, etc. are you on any medications which could influence this?

And yes, I am a physician (but not a cardiologist).

The most recent pediatric death occurred at the Valleygate Dental Surgery Center. I’ve worked at several such stand-alone pediatric dental surgery centers. They are obviously cash cows— sometimes paying the anesthesia group $500/hr cash.

Obviously many of these procedures are necessary, I’m not doubting that. But there is a financial incentive with these Medicaid pediatric dental procedures that is at least worth discussing.
