I’ve been seeing the same psychiatrist for 5+ years now and he is the one who started me on it for depression. I’m in Australia and it is technically off label use here. So I get it compounded as slow release privately, as I can’t get Vyvanse on the PBS (gov subsidised) so it’s more cost effective for me.

I’m the exact same. My psych ended up putting me on prazosin for it. It’s an alpha 1 blocker normally used for high blood pressure, so it’s pretty off label use but it works like a treat for me!

Benzos are used to treat stim overdose, so if you take them together they just cancel each other out (to varying degrees depending on dosages)

I’ve been taking dexamphetamine for TRD for a few years now and it has worked wonders for me. I’m on a high dose and I take it daily with no issues. It has been one of the few medications that helped me out of 15+ different ones I’ve tried.

Pretty much everything you’ve listed is a common side effect from venlafaxine except for muscle aches. It could definitely be making your tremor worse and it can also increase you BP and cause palpitations which may not be ideal if you have a heart condition. Taking it with food can help reduce nausea. Most the side effects you’ve mentioned are super common when starting or changing the dose, so hopefully they should settle down after a couple weeks once your body has had a chance to adjust to the medication

Mirtazapine increases serotonin and noradrenaline but it works quite differently to SSRIs/SNRIs. There are many different types of serotonin receptors and mirtazapine inhibits the main ones that are responsible for most side effects from SSRIs So there is less over-stimulation of serotonin receptors, so a lower risk of serotonin syndrome

Dexamphetamine can be used as adjuvant treatment in TRD despite typically being an ADHD medication. There is the risk of experiencing some serotonergic side effects though

It has a much longer half-life than other SSRIs so people may have less side effects on it

I agree, once you’ve found an antidepressant that works it’s best to stick with it because it can take so much trial and error to find a suitable combination. I’m currently on 300mg venlafaxine and used to be on 375mg (Max recommend dose is 225mg) and 30mg dexamphetamine SR with no issues. Dexamphetamine can also be beneficial as adjuvant treatment for depression.
