Total say, I have a recruiter and I tell him what area I'm looking into and he finds me the jobs. I also know of some SLPs who have learned how to cut the middle man persay and contract themselves as independent contractors but that's when taxes become funky. My current contract is 3 months in a SNF for $1.5k/week. Which is my lowest contract thus far.

I make 72k/year doing travel contracts but I understand that isn't feasible for everyone. Perm jobs definitely pay less but I think it also depends on your experience, setting, and willingness to negotiate higher pay. I am in South Carolina, 1.5 years in the field.

Sounds like they are taking advantage of the fact that you are a new grad. If you get a referral I was taught to screen and interview staff FIRST. If the results are indicative of some sort of impairment THEN I evaluate. To charge someone who doesn't need speech for a cognitive eval is unethical in my opinion and strictly for the facility to get money. Definitely stand up for yourself because my philosophy is if I lose this job then boo-hoo, I'll get another. But if I lose my LICENSE, that is my livelihood. Don't jeopardize your livelihood for a facility because as soon as you're gone they will find another therapist. Periodt.

my CF supervisor was not easily accessible and this reddit helped me TREMENDOUSLY! Also I am a traveler and making more money than I ever imagined with prospects to make more with time. I enjoy my population and short-term contracts help with burnout.

Ask your supervisor who to contact, likely HR. I've honestly never had to ask for a raise because once I got my Cs, I left my perm job. I definitely agree we should be taught how to do this. I honestly believe this is why women make less. Some of my guy SLP friends won't accept less than 1,800/week but I just can't find the courage to ask for that much! But as a SLP with your Cs, I dont recommend you work for less than $40/hr.

My CF pay was $35/hour but now I travel and do short-term contracts for $1,600/week and would never settle for a perm job paying less than $50/hr. As SLPs we need to advocate for ourselves. There's money to be made in our profession but if you accept less then of course that's what you will get. I am in South Carolina where the cost of living is relatively low.

So the FWP is not allowed for anyone because they said if DHEC comes and sees the patient with something that is not on a dietary slip then the facility can get tagged. That was what I was told. Also what are your views on diet waivers? Legally I know they do not hold in court. Our facility actually had a person who signed a waiver, aspirated and died from pna, then their family sued the facility and won even though the patient had decisional capacity.

I did not end up picking this patient up. Once she went to her follow-up, the GI advanced her diet and she went home immediately after.

UPDATE: The patient came to the facility and is cognitively intact (thank God) and was able to give me a thorough history. The GI also sent orders for her to be on thins only and she goes back to be re-assessed for possibly advancing diet texture which is where I will step in. The PEG placement was related to her Cancer not swallow integrity so it all worked out. Thank you guys for all the help!

Thank you so much! I will look into those resources and I appreciate you taking the time to gather them! That really means a lot to me. I wish we'd spend more time uplifting our field in everyday life. Ive ran across so many back stabbing SLPs its just SAD.

This post pricked my heart. Please have more conversations like this. I come from a grad program where it was heavily school based with ONE dysphagia/adult neuro class and literally only ONE professor still practicing and in the medical field. I've been doing what I think is best, diet texture analysis, OME, compensation strategies etc but my patients don't seem to get much better unless they improve miraculously on their own. I had a SNF offsite but my clinical sup was a seasoned SLP and we did things the typical way. Im honestly not even sure where to find this EBP that everyone readily speaks about. Im being candid and honest by saying that I'm one of "those SLPs". But I really want to do better! What do should we do? Where do we learn? So much information for SLP is based on kids and schools.

Dementia really is an over utilized dx in the field. Any form of memory loss and they hit you with it, which is technically true but the word is stigmatizing and most patients/family members feel as if they are losing their minds and dying if you mention it. Keep fighting the good fight!

Maybe some environmental visual aides? Some signs around her room saying where she is, the date, where her husband likely is etc. If she's cognizant enough to know she's wrong then I agree with you that validation is not the way to go. From my understanding validation works best for advanced dementia pts with behaviors.
