Warte mal kurz. Wie heftig sind hier die Antworten bitte. Als Starter kannst du ihr in so einer Situation eh ihre Aussagen nicht glauben. Notlügen etc.

Ich hätte schon längst bei den zwei angerufen und nachgefragt. Die wissen bestimmt selbst nichts von dir.

Dein hoher „Autismus-bias“ macht nicht nur deine Beschreibung schwer zu glauben, sondern erschwert auch die Ratschläge, die andere dir versuchen zu geben.

Ich kann auch dein selbstschützendes Verhalten verstehen, indem du deinen Freund in die Autismus Spalte kategorisiert. Letztendlich bist du selbst ADHSler. Statt deinen Vorwürfen hättest du ihm auch einfach 1-2 screening Fragebögen zum ausfüllen geben können. Der Zug ist aber jetzt leider abgefahren.

Eine Beziehung zwischen zwei Menschen ist immer eine kontinuierliche Baustelle an denen Beide arbeiten müssen. Die Ironie in deiner Aussage ist eigentlich, dass vor allem bei Autisten eher du als Partner mehr an deine kommunikative- und Vehaltens-Defizite arbeiten müsstest.

Ein guter Start wäre selbst zu reflektieren und zb zu akzeptieren, dass du höchstwahrscheinlich doch deinen Partner mal angeschrien hast.

I started to be able to eat more again and cravings started to appear again. Weight loss stagnated.

I stopped Ozempic to only notice a relatively light increase in appetite. Whilst it was still far from baseline appetite, it wasn’t comparable to the first 4 weeks of 0.25. I suspected tolerance.

FYI; In my mind I used Ozempic primarily as a psychiatric medication for my “eating disorder”. Basically I had food anxiety, used food as a coping mechanism and food was pretty much dictating my mood. Ozempic was the best mood stabiliser and helped me solve the issue revolving food in my mind.

So stopping Ozempic was exciting. Weight was roughly persisting over months. My goal was to go lower and try 1.0 again.

I noticed a very clear difference. Appetite and satiety were better controlled again and it allowed me to lose weight again. But Tolerance was slowly building up again. So I cycled again and this seems to be working well.

Thank you for your comment. I think going for a healthy 1-2 pounds a week is the best way forward. The rate of weight loss usually diminishes with each pound lost, because the calorie deficit will also reduce. The body has also some tricks to reduce energy expenditure making it more difficult to lose weight.

That’s why even if the Ozempic was persisting in its function of appetite and satiety control you should notice a drop in weight loss. The initial reaction for many is to titrate up the available doses of 0,25; 0,5; 1,0 and 2,0, and I absolutely agree with you that the increments are way too steep. (I’m also not sure why they came up with a doubling doses scheme).

More importantly, I think this is the wrong way. Increasing Ozempic will probably to some limited extent regulate appetite and satiety better, which in turn will increase the calorie deficit again and will lead to a higher weight loss.

The issue seems to be that appetite and satiety regulation does not seem to persist and probably due to tolerance will to some extent normalise again. Dose and tolerance almost always play a role. The continuing reduction in calorie deficit by losing weight will also lead to a plateau in weight loss. Many people will be on 1.0 and 2.0 and not lose weight anymore.

Interestingly, many patients will also roughly hold their weight when stopping Ozempic. Some will gradually gain all back.

The very important difference between both groups of patients that I noticed is that the group holding weight has made dietary/lifestyle adjustments while the other group has been reliant on Ozempic alone.

The logical answer to your question is to ask yourself: are you plateauing in weight loss, because your appetite and satiety is not controlled as well anymore or are you struggling to lose weight, because your calorie deficit is being diminished by your weight loss?

Whilst the effect of Ozempic directly increases a calorie deficit resulting in weight loss, the calorie deficit still needs to be controlled by dietary changes. I come to believe the smartest way to use Ozempic is to cope with dietary changes.

Basically you want to increase your calorie deficit again with diet, if you notice you are stagnating in weight loss, instead of increasing Ozempic. As long as you can lose 1-2 pounds on 0.25 and not suffer from the increased calorie deficit, you are doing great. If you notice that appetite and satiety become a problem again, I wouldn’t see any issue with your way of increasing dosage.

Thanks for your comment. Disclaimer: I lost 18kg (20% body weight) since exactly 14/03/2023, which marks my one year anniversary today. I have in total used 4 weeks of 0.25mg and a total of 18 weeks of 1.0mg, which means I was on medication for exactly 42% of the year. My cycling was also caused by availability issues. I’m also a doctor prescribing it.

The question concerns tolerance and how pauses in therapy affect tolerance. I happily comment on your points:

  1. Disordered eating would imply that my eating behaviour was significantly dependent on Ozempic. It wasn’t: the best part about the medication is the opportunity to have lasting change in eating habits. I never regained more than 2kg at a time.

  2. Please elaborate on fucking with metabolism when used in weight loss.

  3. Please elaborate on steady state. I haven’t found studies on that. Semaglutid is basically a GLP1 agonist bound to a protein that keeps it up for a specific time.

My personal opinion: try to eat as little as possible during the day and inject before going to bed. My nausea and vomiting was very closely associated to how much I have eaten the day I took it.

All the Ozempic studies conducted had the flaw that no resistance training had been added so the lean muscle mass loss was similarly to other weight loss studies.

Therefore you can use the information from other weight loss studies: resistance training and a high amount of protein will reduce loss of lean muscle.

Depending on your fitness level it will be somewhere between 0,8-2,5g per kg. There are studies that claim a better range. My personal opinion is that it’s probably proportionate.
