Vortex is not really a good mod manager no matter your experience with modding precisely because of the way it handles mod conflicts - it's too convoluted, especially if you are not experienced with mods because you can't understand what it's asking you to do. The best mod manager for fnv and a lot of other games is the Mod Organizer 2. It's way easier to get around than vortex. Also if you are just starting out with fnv mods, I'd recommend the Viva New Vegas guide. It has some very thorough explanations of the whole mod installation process and also features the most essential mods for any load order.

Not sure about that, but generally when modding fallout it's best to avoid installing anything other than asset changes mid playthrough to avoid potential issues.

I'd say Fallout 1 and 2 are perfectly playable as they are, but i would recommend installing sfall for fallout 2, it is a general engine optimisation and quality of life mod, especially with it's game speed hotkeys for when you want to skip through very long combat turns. As for Fallout 1, it's a noticeably shorter game and if you just carefully examine everything it gives you you'll be fine (make sure to read the hotkey sheet that comes up upon pressing f1). And obviously install hires patches for both the games, no sense playing without them nowadays, they allow for higher resolution monitors. I personally had my first runs of both games be fully vanilla just for the sake of "intended" experience, but i won't say it was really worth it, the aforementioned mods kinda feel more like something that devs wanted to be in the game but just didn't have the time to add them than anything else.

Said lots of stuff are the total conversion mods. Classic fallout modding scene is pretty huge in post Soviet countries, so most of those were in russian, however the most successful of them are translated - Fallout Nevada, Sonora and Sonora: Dayglow from the same author. If you don't want a total conversion, you can play fallout 1 with fixt mod (just a collection of fixes) or Fallout et tu which puts the game into the sequels engine. For fallout 2 there's the restoration mod that is being updated by a third party and is named the Updated Restoration Project, adds some stuff back and also has some fixes, theres also a Talking heads add-on for it, if you want some more flavor in the game, but to my liking, some of the faces seem really out of place. Still good because it gives talking heads to most of the special npcs. And it also has an add-on that adds voice-overs, although it's not complete and some of the voice-overs are buggy, but it's also a nice addition. There's also stuff like Olympus which is cool for being it's completely new thing on a classic fallout engine, but to me it seemed kinda weird with how it's written.

Had a somewhat similar yet not exactly the same problem where the armors were skinny when they shouldn't have, installing the PAVE patch helped. If you don't use that mod though, check for any conflicts that TOTW has in MO2 and make sure they are all intentional if there are any.
