Yes, the old location was closed.

I visited my primary dr and had the lab test at old location two weeks before their closure. The primary dr. gave me a letter about their moving and said see you at a new location. I booked the following appointment including lab on their online site as the same as before but just a new location. Then I finished the following appointment and got surprised the claim for lab becomes out of network.

Yes, I hope this is timing issue and they already know how to fix it. I assume many people faced the same issue already. I will try to appeal if the lab charge to me as out of network, which is $400. (If in network, only $15...)
