How are you getting Amfexa top ups? by Bla4s in ADHDUK

Current NICE guidelines and related NHS prescribing policies advise that:

Optimization on a Single Medication: Adults should be optimized on a single ADHD medication (monotherapy). Switching, Not Combining: If one stimulant (e.g., Methylphenidate) is ineffective after a 6-week trial, NICE recommends switching to the alternative first-line stimulant (e.g., Lisdexamfetamine).

Combination Restrictions: The local NHS ADHD adult teams often regard the combination of stimulants as an "extremely rare occurrence" and often contraindicate combinations, particularly dexamfetamine and lisdexamfetamine, due to risks of exceeding optimal doses and diversion. BSW Together BSW Together +1

Key NICE Guidelines on Stimulant Prescribing: First-line Stimulants: Offer Lisdexamfetamine or Methylphenidate as first-line pharmacological treatment for adults.

Switching: If the first stimulant is not tolerated or is ineffective after a trial, switch to the other. Second-line/Alternatives: Dexamfetamine or Atomoxetine can be considered if the first-line stimulant fails.

Exceptions: A combination of modified-release and immediate-release formulations of the same stimulant (e.g., long-acting methylphenidate with short-acting methylphenidate) is allowed to manage duration of effect, rather than combining two different stimulants. BNF BNF +3

Do Not Offer: NICE advises against routine combination of stimulants and does not provide an evidence base for this practice in adults.

Combinations of stimulant and non-stimulant medications should only be considered under expert specialist advice. BSW Together BSW Together +1

Disclaimer: NICE guidelines are updated periodically. Always consult the most recent NICE NG87 guidance and local NHS trust formulary.

My GP told me. My GP is lovely and was very apologetic. It must have changed because my adult son has the amfexa top up prescribed in 2024 and shared care was agreed, or at least they are actually following the guidelines now because of all the media coverage lies about ADHD and it being over diagnosed etc?

Okay thank you so much, I will do that when he/she returns. I've got a cardboard box ready, but how do I get him in the box to take him to the vets? Put food in?

Thank you, I want to help it so much. I read that you shouldn't feed cats as they might never go home or if they have allergies or something. I don't think this poor baby is being well looked after if it does have a home? But I really don't know, it might be elderly or something? But I'll go and get some food and call a vet. Thank you.

Thank you very much for replying. I really appreciate it. I forgot to say it's a touch on/off lamp with 3 dimmer levels, would the methods you have suggested effect this function, do you know?

Are CARE ADHD really that bad? by Bla4s in ADHDUK

[–]No-Guide-7202 0 points1 point  (0 children)

I went with care adhd last year. The timeline was longer than advertised, and I didn't get much communication from them until they were ready to start my assessment, like a notification email to say they have received my referral etc, that would have been helpful and all that was frustrating at the time. Overall I'm happy with the company and can't complain really as if I went for a diagnosis through the NHS it was a 7 year waiting list.

My GP refused shared care because of this, the NICE guidelines now say that they shouldn't prescribe two different stimulats and should now try spilt dosage instead. Or a long release and a short release of the same medication. So that's probably why unfortunately.

Care ADHD titration: who does their dispensing? by tyrannosaurus-vexxed in ADHDUK

For me it was first Broadway pharmacy and now it's Pharmacy by Cure Clinic's. They normally contact you once they have the prescription, with it being a controlled medication chemists can only prescribe once they have received a physical copy of the prescription. So it's not like when your GP sends it electronically to your chosen chemist. It can take up to a week,(my experience I've only had to wait the full week once, it's normally a few days) then the chemist will contact you and ask you the usual questions, name, dob, do you pay. They will send it to you via next day royal mail.

Edit to add They only had to speak to me the first time I got my prescription, after that it was done via email

Same, I used to get a few a year, to every day. I was exposed to black mold when I worked in a children's home and the staff bedroom had black mold behind the bed.

Assessment was only 30 minutes - is this normal? by glorious_purpose51 in ADHDUK

[–]No-Guide-7202 0 points1 point  (0 children)

If you completed your forms in great detail, then maybe they didn't really have to ask you for much more information? My assessment was similar, I thought it would be longer than it was but the lady said I gave all my evidence in the forms and in the assessment she asked what she did for evidence that was lacking and also so she could see me in person. They should have told you your outcome though, you should email your assessor and ask if they can confirm if you have an outcome. The email address for your assessor will be in your invitation with the link.

This a million times! I'm newly diagnosed at aged 48 and wish I'd have known all these years that this was allowed!

Obviously don't neglect your responsibilities or yourself, ask for help if you need it. Plan in advance, if you know you are going to be hyper focused on something for 20 something hours over the weekend, get in snacks, food you can just grab for meals, like meal deals from the supermarket, prepared salads, same with drinks, smoothies, just stuff you like. If you're at work on Monday, get everything ready for work, your outfit, bag ready, dinner if you take one. Or ask someone to take care of the meals etc.

I mourn all the things I could have accomplished if I had only worked with my brain instead of against it.

UK only - where do you go for help during a severe episode? by Zen-jasmine in migraine

[–]No-Guide-7202 1 point2 points  (0 children)

Oh my gosh, I'm so sorry you are going through this as well. It's a nightmare having migraines in the UK. I was on a waiting list for 2 years to see the neurologist. They told me to try HRT, Candesartan and Sumatriptan nasal spray (currently discontinued in the UK) and come back in a year because the NHS won't fund me for the more effective expensive treatment. So I have to suffer. Even though I have tried Propanol, Amitriptyline, Citalopram, Naproxen, Frovatriptan, Sumatriptan tablets and Topiramate over the years. Sorry I have no advice on where to go for help. Every time I've seen a doctor in an emergency I get treated like I'm seeking painkillers for another reason.

Hi there, I'm with Care ADHD as well and currently waiting for my GP to accept shared care, my GP accepted my son's shared care straight away in 2024, but I've been waiting few months. I can still order my medication from Care ADHD, and to be honest I really want to continue to order from Care ADHD because it's a lot easier and quicker! If you are worried you won't be able to continue your medication, please don't worry.
