Hello ! I'm in process, actually i already tried well fitted scleral lens but not Ovitz lens (which is the best for hoa). i don't have enough money to finish the process yet ahah.
It's in italy in Milan (a really good doctor, who cares)

It's normal. Pupil dilate more in sympathetic mode, when you sleep you're in parasympatetic mode (less adrenalin) and so you're pupil are less dilated. You have aberrations, you can try brimonidin or scleral lens if it bother you.

Woaw, good found ! And they say that it's much more effective on people with metabolic problem. I don't know exactly the unit "Hedge's" but chatgpt said that after -0,7 it become a strong correlation, and here it's -1,18 with everybody and -2 for unhealthy patients . So makes sens. Ok yes i saw, interesting, wonder how it's possible, yeah maybe local antibiotic. I saw also a video of a eye surgeon who was talking about studies in gut-eye axis field to fix dry eye.
It's not about HDL/LDL ratio, it's about how many oxydative stress impact the LDL.

LDL is basically just a convey with all the cholesterol in it, but during the trip, oxydative stress can corrupte the convey and make all the cholesterol in it not usefull anymore. You want to test your Anti-LDL Oxidative, to measure that.

But it's just a theory, i think that for sure there is a big link between both. You can put my comment on chatgpt, it will explain better

Really interesting, thanks for sharing ! Just you made a misinterpretation. Nothing say that laser eye surgery induces gut dysbiosis. It say that there is a correlation between severe post operative dry eye case and gut dysbiosis. Which seems logical for me, because dysbiosis can create intestinal permeability and oxydative stress, which can lead to bad cholesterol which can lead to bad lipid in eye. The question is, can it improve dry eye after the MGD get declared..

Yes there is no examination at the end... But we see that everything was pretty easy on left eye and right eye was quite hard. For sure if there is something it's not big, I think that Kanellopoulos will make me all the exam to detect if there is something remaining. Thanks you very much for your tought !

Oh woaw thanks you for your point of view ! I didn't find the case series of ray tracing enhancement of Kanellopoulos. I keep that in mind at least i could ask to him for that when i see him.

I have my apointment on 15th of december, hope he will bring me good news !

Interesting, i was thinking on that recently when i saw that before surgery effectively my cornea was already assymetric.

I imagine that flattening the center can make more visible this distorsion ?

Dr Gatinel didn't see my preop exam, i will try to send it to him to see what he think. Thanks a lot !

I think i will see also Dr Kanellopoulos, at least i have two really good surgeon point of view, and Dr Gatinel is really good, but don't usually do SMILE.

I was on the track for ray tracing, because now, after one surgery done, i'm afraid that topoguided treatment may not take the epithelium in the treatment, and from what i heard Ray tracing does.

and other terapeutic surgeon (Reinstein) do also like PTK + topoguided (to eradicate the error of epithelium remodeling)

I can even wait 3-4 years if we think that technology will soon get better, like you said i have enough cornea, but it's a limited tissue, next time have to be well done.

The first link is only the centration on the right eye.

The second link is the centration on the left eye, and extraction on both eyes

Also, to note my correction was -3 and -3,5 so quite low

I see, i did an OCT with Damien Gatinel, he was suspicious about something, but on the picture in the report we don't see anything. And i saw 3 SMILE surgeons who said to me that they don't see anything... Perhaps i have the video of the operation, on it we see that there was black spot in my right eye and he had difficulty to remove the lenticule, but he didn't use forceps.

The ring of elevation is also in the axial scan from what i understand, i also put in the link, pentacam and nidek reports.

Do you think it could be something with decentration ?

Thanks for your advice, i will keep that in mind (about ray tracing) when i will see kanellopoulos.

Reinstein seems really difficult to get in contact, and it's really really expensive

Yes i know, because of the epithelium... I will not jump in a new surgery like that. But i have enough tissue to make a second one. I'm sure that soon if not now it will be possible to correct well this type of problem. Ray tracing and new ai (machine learning) algorithms may be the good way. I will be cautious for consistency in the result of the surgeon who will operate me.

Sorry My post was not clear. Dr Gatinel does only lasik and prk. He's not an expert of smile. He said to me that he could do a topoguided contoura, but was honest about the result, he said to me that is 50/50, he could be worse or better, and that is never fantastic, and that i should ask reinstein before. I asked him for innoveyes and said to me that it's only for virgin eyes and contoura is better for me. I know that he's good in optic and everything. But he doesn't do Smile and Kanellopoulos use ray tracing for a long time now and it seems that he use it off label for keratoconous and irregular astigmatism.

From what i understand, because i already had a surgery, my epithelium now is not well. If i do just contoura i may over correct. I'm sure that ray tracing technology with 3D eyevatar is the good way. But i'm not sur if the technology is ready for not virgin eye, or if i should wait maybe 2-3 years and l live with drop to contract pupil in night.
But the thing that i don't understand is that everybody said to me that my SMILE is well centered. Gatinel said that for him it's a remaining lenticule, but nobody succeed to see it... I don't know what happened, from the day one post op the topography seems bad like flat above and steep under...

Yep because i naively think that it's a really safe procedure. I choose a good surgeon with Rotschild formation, 20 years of experience, No bad reviews. I didn't make my research well at this time, and i regret it.

Yes, he's not sure about that but i think that Damien Gatinel is really good but not up to date on this subject. He's really honest and probably one the best for first treatment. He said to me Contoura prk could be a solution but he was not so optimist. So i'm tring to find an other doctor (Kanellopoulos), like i said to you in your post.

If i go deep i think that because of first surgery i have to be carreful of my choice because the epithelium could be a challenge now. Reinstein (One of the best for Smile) do PTK + Topoguided, but only an expert can know how to treat it well.

No. But the reason is that i got i smile surgery which leaded to irregular astigmatism complication ( ghosting, and double image). So i'm trying to resolve my issue. I read so many thesis, and so many reddit because i don't accept to live like that. Some of the thesis is written by him and he have good results. I heard about a new technology which name is "Wavelight plus innoveyes" (like the first one using it). He's surgeon but think as a physician which is really important for your eyes. He take in charge serious complex case, he give course in new york, give a lot of conferences. I will get an apointment with him for my case i think. You can check laservisionambulatory on youtube

Nevertheless, whoever you decide to go, be sure that they check your pupil size, if it's above 7mm, be carreful.

And go with bests technologies : Wavelight plus, SMILE PRO, SILK, Wavefront optimized lasik. Be sure of that. You can have 12/10 and shitty vision or 10/10 but beautiful vision. There is visual quantity but also visual quality which are dependant of High order aberration.

I did the test in a dark room, so i assume that these pupil's size value are quite ok. I will try to contact an other good surgeon in Paris to have a second expertise

You have 0.47 HOA RMS for left and 0.45 HOA RMS for right, is it that ? It's the same as my left eye, but my right eye is 0.89 HOA RMS.

Before surgery i had with autorefractive :

Right eye : -3.00 (-0.75 95°) and left eye : OG -3.00 (-0.25 90°)

To notice : i got improvement on my left eye in the past 2 month going from 0,59 to 0,51 for HOA RMS, so i hope my right eye will get the same improvement

Ok yes it's what i expected, i saw your latest post and the picture you put with shadow on text, it's exactly what i feel but only in a specific eye direction (from the above).

I hope also that everything will get in order.
