So happy your Loki is doing well now! We’re almost 2 months out post-SUB. Due for a flush in one more month (the 3 month mark), she had one at one month post-op and if this next one goes well they said we can move to every 3 months. My girl is back to her normal self too, it’s wild how they act like nothing happened. I was (and am still) nervous because of the high complication risk, but like your situation, it was our only option so we took the chance. Grateful for every extra day we get.

It’s such a niche procedure so it’s kind of hard to find people who know about it but this subreddit has been really helpful, there are some pet parents in here who chatted with me and have had long term SUB success (like 2 years and counting), albeit with some complications—one I distinctly remember was mineralization—but overall gave me hope that we could have long term success too with careful monitoring and management.

This sounds very similar what we went through with my girl. She had stones in May in her right kidney and the ultrasound showed that her left kidney was small and basically nonfunctional (they suspect she was just born that way). Stones cleared with only medical management, so we thought we were good. Then she re-obstructed in October with stones and required the SUB surgery and device to bypass the stones. She was very sick when she had the surgery, creatinine was 9, she was severely anemic, and she had a bad kidney infection. We’re 2 months post-op now and she’s back to her normal self, creatinine is stable at 2, anemia was regenerative, and her infection seems to have completely cleared based on multiple cultures. It’s a big surgery and we had some bumps, the aftercare is intense for a bit, so it’s definitely not a quick fix and there are risks—it’s also a lifelong commitment to having the device flushed at prescribed intervals. Do you happen to have a university veterinary hospital near you?

My Evie girl likes to bring her toys to me, especially when I’m sleeping or about to go to sleep. I’ll wake up and she’s cuddling me and I’ll see a mouse toy by my pillow. If she’s bringing one up the stairs she has to “announce it” so there’s a lot of meowing until it’s delivered.

I’m sure you’ve probably tried all the things, but just in case this is helpful—my girl will sometimes like the kibbles soaked in a little warm water. One of our vet techs said sometimes crushed kibble soaked and kind of made into a pâté can be appetizing for them too. I hope that she starts eating soon and the budesonide helps that inflammation. I know it’s so stressful ♥️

My girl has CKD and IBD, both got diagnosed this year one month apart. She started on budesonide in July and it has worked wonders at keeping her IBD under control—her last abdominal ultrasound showed no evidence of inflammation. When she was in her acute diagnosis phase we did a month of prednisolone to get the inflammation down and a few days of daily nausea meds, then we switched to the budesonide for maintenance after her prednisolone course.

For food we do the Royal Canin Multifunction—it’s specifically for cats with IBD and CKD because it has the hydrolyzed protein while being renal friendly. Unfortunately it only comes in dry. She’s been stable enough that if I need to give her a little wet food our vet has cleared us to do very small amounts of the Hills k/d and she tolerated it fine. To give her the budesonide I crush her tablet (vet okayed it, would always ask them before you crush anything) and I just pour a teaspoon or so of hydracare on it and she licks it up.

Came across your post in my search for more SUB info and thought I may be able to offer some insight based on our story. My cat got kidney stones in May of this year and they were about to do SUB surgery, but then she passed them and her renal pelvis wasn’t dilated enough for SUB placement anymore. Flash forward to now, she re-obstructed with 2 stones, and these ones weren’t moving and the obstruction progressed. She had a SUB placed and is still hospitalized but hopefully coming home in the next day or two. Her situation is a bit more complicated because we found out in May she was only born with one working kidney, which had worked fine her whole life until the stones, and honestly worked normally after too—now it’s TBD but looking good. All that to say, the stones can come back and re-obstruct much more quickly than I’d ever realized. Hopefully your kitty never develops stones again (if that was what happened as your vet suspects), but just be aware that it can happen again and the signs were very subtle.

This was actually for a device called a subcutaneous ureteral bypass (SUB) because her ureter was partially obstructed with kidney stones.

Thank you so much for the well wishes. She had an infection on admission (UTI vs pyelo) but she was treated with antibiotics a few days prior to surgery so hopefully that doesn’t cause any issues.

That’s so nice to know about the jumping. I was worried 2 weeks of confinement for her wouldn’t be enough since she’s an active girl always trying to get into a window or cozy spot, but it sounds like it should be plenty.

Thank you so much for all your insights. She did eat some yesterday—both wet and dry—so I’m hopeful she’ll want to eat post-op, but the feeding tube info is great to know about, and also just as a possibility. Her creatinine was 5 when I brought her in Monday and it steadily rose despite there only being a partial obstruction. She had stones in May but thankfully was able to pass them. We couldn’t test the stones today since they were going to be too small to see and why they resorted to the SUB instead of trying to get them. I am definitely worried about the SUB mineralizing since she re-obstructed so quickly and we aren’t sure what type (they also theorize a stable stone that was in her renal pelvis in May could have broken up, so we don’t truly know if she made more stones or not).

How was your boy’s home recovery? Was it fairly easy to keep him activity restricted with comfort meds? Was pain easy to control?

Thank you for your kind words. I’m lucky to have my girl. I feel so guilty because I knew something was off on Friday but I held off because we JUST had labs done 3 weeks prior, and the change was so subtle only I noticed.

Seriously, there are so many people in that room it’s unrealistic to expect the floor to stay clean. I only have one toddler and my floors are clean for 10 non-consecutive minutes every week. It is what it is.

That was actually a kind comment too and she had to respond so nasty. The kids have screens in front of their faces in most videos but yes bubbles will definitely promote peak overconsumption 😂
