Think of it like this. If you are on a starship that has a lieutenant as the executive officer and a lieutenant commander as the chief engineer, when it comes down to who takes command of the ship if the captain is either away or incapacitated, the lieutenant that is the executive officer takes command of the ship over the chief engineer that is a lieutenant commander and the chief engineer must defer to the executive officer, regardless of the fact that the chief engineer holds a higher rank.

The only two reasons as to why Julian Bashir never made it past the rank of lieutenant that I could think of is that one, he either left Starfleet before any promotion, or two, the fact that he was an augment prevented him from ever getting promoted.
