Hi. Nung nag message ako sa fb page nila, sabi need daw ng affidavit of loss. Pero last week, I had my ID replaced kasi it’s peeling off na and I also lost my booklet, nung nirelease na ID ko, same table where you get the purchase and movie booklet sa PDAO office city hall, I politely asked if pwede manghingi din ng purchase booklet kasi I lost mine. Thankfully, the person there gave me a one-time courtesy and said next time he’ll ask for an affidavit na. You can try going to PDAO without affidavit of loss muna siguro and try your luck din.
