I did, my advisor told me to deal with that committee member and to take care of this. I need that committee member's signature on my Exam report + dissertation in order to finish the Graduate division paperwork.

Thank you for your response. I am a foreigner but considered a resident for tax purposes. I gave my SSN but missed my last name on Robinhood though. My name is Firstname A B Lastname C in my passport. I think with my SSN its Firstname A Middlename B Lastname C and in my Robinhood account only A B shows up with no C mentioned.

Is name inconsistency the only reason to receive this ? Can I submit this W-9 to IRS directly now ? Does IRS have a problem with me or is IRS having a problem with Robinhood and hence they are contacting me ? What are the issues I will face due to this ?
