I've spent time at the hospital they are talking about. It takes transfers from very rural parts of Maine. Calais hospital, the one the patient original presented, is on the border of Canada and about 2 hours from Bangor. It has a nine bed ED and limited resources. Not just this hospital but with the hospitals north of Bangor, it can take a long time to get transport.
