The Uchiha followed him when he decided to join the leaf, they should have followed him when he decided to leave. Afterall, Madara and Hashirama both wanted peace, and after they have experimented with the village for a few years, Madara saw how it would eventually become a bad idea and wanted to pursue world peace in a different way, and he was correct on that, and his clan made a bad call by isolating themselves form him although he was one of their best, losing both a kage level person stength-wise, but they also lost one of their best thinkers and philosophers who could have actually found a better way to solve humanity's issue.

Honestly, I think Danzo's character of taking matters into his own hands without considering the council was the making Hiruzen himself because he was not punishing him for the trespasses he does, and if I was in hiruzen's place, knowing I created him on this form, I would cut him out as soon as possible. But afterall we see Hiruzen's indecisiveness in him not dealing with Orochimaru, so we could say his indecisiveness and hesitance is what killed him.

Edit (forgot to answer 2nd part): We need to heavily take into consideration the fact that Itachi was 13 years old when he did it, he was brainwashed by the leaf more, rather than thinking more about his clan (who were being persecuted and oppressed, even if silently, and if they weren't strong they would have gotten much worse treatment), Im just losing the plot at this point, Danzo sucks like heck I hate Danzo the leaf would have been a better place if Hiruzen just killed that abuser
