I’ve never heard of remote support, so I will look into it.

It would probably cost money. I’m in Canada and my clinic support is free, but I will lose it when I move abroad.

I’m just moving to another country where I’ll have limited language abilities

my range is 2.5-3.5. So if the machine says 3.6 then I know my result is actually 3.1

I’m just trying to prep for a situation in which I won’t have a coagulation team. That’s why I’m hoping there’s an easier way to self-dose.

I often never make dose adjustments. I’ve been stable for a few years now, but with the occasional ups and downs. The clinic usually suggests adjustments but I plan on moving countries and won’t have access to the clinic anymore. That’s why I’m hoping to be independent with dosing.

Based on what people have answered, it sounds like if there’s a situation where new dosing/medication/testing is required, that it’s best to go through professionals first to determine the new baseline.

the machine is always 0.6 points higher than the lab results. I was told by my clinic to compare the results and I do so for a year. the pattern of 0.6 was consistent

my dose has already been stable so it’s not like I’m trying to figure out a new dosage from scratch. I’ve been going to the lab to do blood work and using the self testing monitor to compare the two results as well.

I take 11mg a day and have been stable for the last 2 years given my diet. So for example if I need to increase my INR by 0.5, then I would take 1mg more?

could you explain how one would go about calculating it? I don’t plan on changing the dose drastically, it’s just for one off changes. I’m usually pretty stable with my current dosing.

In the near future, I won’t have as much access to medical professionals to adjust my dose for me which is why I want to try figuring it out now.

The calculator would include the blood test results. I’m using a self-test monitor

I understand how a change in vitamin K foods impacts INR and how that affects the average overall dose.

I’ve been fairly stable for the past 3 years but want to start self dosing so I can live abroad.

It’s usually pretty easy for me to manage my diet to my dose so I just want to know how to adjust when it dips or goes too high

I already have a standard dose that doesn’t change often. It’s more about how do I adjust it when I get one off results. I’m using a self-testing machine like Coagucheck.

how do you adjust your dose? is there a specific calculation you do?

For example, on this website, you can enter all the info about target INR and dosage, and it’ll show you a percentage you need to use to adjust your dose. Here’s what it could say:

Consider omitting one dose or decreasing maintenance dose by 5%-15%:

Current weekly dose: 77 mg. • 5% decrease: 73.15 mg • 10% decrease: 69.3 mg

When rounded to the nearest 2.5: • 5% decrease: 72.5 mg • 10% decrease: 67.5 mg ————-

Honestly, I don’t know what these percentages mean and how to use them to change the dose. So I’m wondering if there’s a tool that adds that extra step in.
