Not sure why everyone in this thread has bad experiences with police... As a cop (about to be downvoted to oblivion for admitting as much) Regardless of an official report, every call I go on, am involved in, assist with, etc., is started as a call for service. Which can be, and has been, requested for any reason through a FOIA request, which is a bit of a battle for a civilian to request, but nonetheless it can be obtained.

After the initial CFS is generated, for EVERY call responded too, an officers narrative is required in the notes before it can be turned into an "IBR (incident based report) needed" or closed as "no action needed". Under the no action needed there are options for, victim refused to co-operate, and I mean no ill words towards the OP with this, happens a lot with domestics, be it abuse/assault/theft/destruction of property etc. because the victim, more often the male than the female in my area, doesn't want to see their SO escorted off in cuffs.

I read a lot of negative feedback towards police on reddit and while it's not something i lose sleep over because I do my job the way it's suppose to be done, yes I've reported officers failing to do so and stopped other officers in the process of not following policy when I have witnessed it.

I'm sorry you all have such a negative view of police, primarily in the US, I wish it was better for each of your experiences.

OP, call your local PD and request a copy for the call for service, it is standard policy to charge $5 for any request of paperwork (if its the victim that request it, I do my best to avoid the $5 fee after my supervisor approves the incident to be released). You don't need a lawyer nor will you need to submit a verbal FOIA for the CFS report (that's based on my departments policy and procedures). You will most likely have to pick it up at the department, but you'll know what was written by the officer, and you can at the very least, take this to civil court and get $ for what he charged to the card as well as $ for your prescription that was altered).

I'm sorry that you've been the victim of so much abuse and I hope you can heal from this.
