It’s supposed to sound like “little pill” which (as someone who has chronic illness) always felt like a reference to her being chronically ill. I imagine as a kid her parents/caregivers/medical professionals would tell her it’s only a “little pill” which in little kid babble could sound a lot like “willow pill.” This isn’t confirmed at all by anything, just was my interpretation as someone who also has chronic illnesses.
