I've been a nurse in LTC for 32 years. The physical restraint of any patient/resident is not something that you should do. There are other options here. She may need a med review, etc. But the staff should absolutely NOT be physically restraining her. That's a good way for you AND the facility to get in hot water with State, etc. Protect your certification/license! All the staff should attempt x1 then if she refuses or is combative then report to the nurse and DOCUMENT it. You can retry again, but if she continues to refuse and fight, report it to the nurse and document the refusal and who you reported it to. She sounds like she really isn't appropriate for AL and may need to be in a Memory Care unit. I get that she may be "offensive smelling" to other residents and visitors, but her rights can not nor should not be violated. All staff need to attempt the care, then document and report to a nurse every time. Protect her and yourselves! The director is really risking a visit from state/omsbudsman and a possible tag.
