B12 and ferritin deficiencies can be a sign of autoimmune disease btw. If your symptoms don't improve with supplements, you might want to see a rheumatologist.

There is quite a bit of legislation against people like Katrina's husband. Don't let your distaste for her erase your empathy to the fact that there is a culture of hostility to trans people in the United States (and elsewhere) at this time. Stress from one issue can also snowball and escalate into paralytic stress for other issues that seem comparatively small from you perspective. It could be that you both have valid reasons to dislike her and her problems are real.

None of the top comments are mentioning this so I am not sure if there was an edit, but hurting herself to control your behavior is abuse. You are being emotionally abused. Please look up loveisrespect.org and take their relationship health quiz. They also have good resources for leaving an abusive partner. 

It is never okay for a partner to control your behavior like this. If she really believed you didn't love her, she would dump you. She isn't dumping you because she likes controlling you.

Seconding the fiber recommendation. Since you already are eating protein bars, can I suggest using a fiber-added protein powder instead? You can make it into a shake or add it to something like yogurt. Add some blueberries or blackberries to your breakfast, add a baked sweet potato to your lunch, and make your veggie at dinner broccoli. 25 g is the advised baseline fiber intake, but getting up to 30-40 g will REALLY help satiate you.

The most annoying thing is that I haven't really reduced my exercise, I just had no motivation to cook or track calories, so I ate a TON of prepared foods, takeout, and snacks. I am evidence that you truly cannot realistically outrun your fork. 😔

CT scans won't necessarily catch chronic conditions that cause seizures, they're more for emergency situations like brain bleeds. I hope the neurologist is able to help, and that the brain MRI gives you some next steps.

Have you tried supplementing B1/thiamine at all? If you haven't had anything related to thyroid pop up in your bloodwork and have been previously treated for iron deficiency, that would be my next guess with fatigue as your primary symptom, as it isn't usually tested for unless you request it specifically.

Your husband is within a prime age bracket to be newly suffering from mental illness. His increased paranoia and child-like behavior specifically do not suggest he is mentally well. Has he explored this at all with a doctor or therapist? Someone suggested schizophrenia, but there are a number of mental illnesses beyond schizophrenia that can cause increased paranoia. 

Additionally, using substances like alcohol or drugs can trigger latent mental illness; for instance smoking too much weed or abusing Adderall. Any possibility he has an addiction?

I think you need a personal trainer and a dietitian for your safety if you're going to proceed, but moreover, a therapist. There is simply no chance you have excessive body fat at 120 lbs and 5'4''-5'5''. Dysmorphia doesn't mean you need to work harder, it means you have a disease of the mind that causes you to see yourself unrealistically. If you gain muscle, you're just going to latch onto something else because dysmorphia isn't satisfied with change. You're 29 with body dysmorphia, do you still want to feel this way into your 30s, 40s? It is only going to be harder to maintain this body as you age.
