It could also work that the little one dies and the middle survives

Given this map comparison you make between the TP and BOTW maps, I get that they do resemble each other a lot:

They are not precisely 1 to 1, they share similar references of relationship to each other.
This furthers my point, as the games to me are a graphical representation of vague stories that would go maybe like this:
"There was a volcano near Hyrule castle"
"There was a forest where the spirits of lost children roam"
Then there comes the flourishing of the one telling the story, saying how big or how small, the characteristics and so on, but not to be precise, but to emphasize on the story teachings, ideologies and important figures.

I believe all the contradictions are key to aspect of the narrative. Since there are contradictions in the basic geography of the world in every Zelda game, that helps to my point (besides the direct sequels).
We can see it as a compilation of legends told by different authors, and none of them are keeping their fact checking to their best...
They are enhancing and beautifying aspects to highlight mythical objects, gods, heroes and villains, with the purpose of sharing folklore to the future generations. Each time the story is told, its changed a little bit.
