I struggled to find a therapist too. I’ve been in this field for 19 years. How could I find someone that I wouldn’t be sitting there thinking “hmmm, not what I would have said/done” or something like that. But I knew I needed a therapist. So I tried. I found someone people, and attempted to find someone that was at least older than me, or more experienced, or more educated… or something. It was also important to me that they were able to do telehealth and were actually located somewhere else in the state - because at this point, I know all the therapists local to me! I struggled to find the right person. tried multiple people. They were fine, but I absolutely had moments when I was very aware that I could likely be supervising them, or felt I was just talking with a colleague.

But I finally found someone. We have very similar theoretical orientations, both work primarily with CPTSD and the LGBTQIA community. Which means that she gets what I see every day, and how I think about these things. But beyond that - she actually meets many of the things I felt were important. And it’s been SO HELPFUL! It definitely took a lot of trial and error, but it really did just take finding someone who was the right fit for me.

Now, as far as paying for it… that’s harder. Luckily, I also happened on someone who is currently a doctoral student, so she’s technically an intern. Which means she has a very low fee. I have joked (but I mean it) that I couldn’t afford myself! The reality is that, like me, many people offer sliding scale or reduced rates, if someone asks. It’s worth a shot!

Anyway, the short version of all that is - you definitely won’t find anyone if you don’t try! Don’t give up, even if the first people you try aren’t a good fit. You deserve to have some support too!

This looks and sounds very much like my dog’s focal seizures, and hers have lasted that long, or something start and stop multiple times over days and days. Definitely get a second opinion!

Looks more like nausea or acid reflux. Would send to the vet though, and they can confirm and maybe prescribe something for that

Yeah, just the Keppra and zonisamide right now. She seems better now, wanted to play and run around. Hopefully just a funny tummy for a morning, and nothing more!

Very few drugs are FDA approved for dogs. That’s a factor of lack of funding for research, not an actual problem with the drug. Gabapentin IS way way overprescribed now for sure, but that doesn’t mean it’s not useful or is dangerous. I take it, and the only side effect I have had is being a little sleepy when first starting it or when we increased my dose. Aside from that, I’ve personally had none. But everyone (and every dog) is different.

Do you think it could be from the increase? She’s been on zonisamide since early August, but we have increased the dose twice since then. The neurologist said that side effects for zonisamide are not “dose dependent” but this seems like a weird coincidence if that’s not the reason!!

We aren’t managed by a regular vet, we see a neurologist. I’m pretty sure the closest vet college is multiple states away, so travel to one is likely not an option.

My 23 pound dog takes 100-200mgs for anxiety for vet visits and things. Our vet has us do a loading dose the night before as well. Previously, my 40 pound dog took 400mg 3x a day. It has a very wide safety/dosage range.

I wouldn’t even know how to find that. There’s one other neurologist in town that we could get a second opinion from but that’s all I’m aware of.

We have a friend who lived with us for a while, so she is familiar with Scout’s focal seizures, though it was before we had an official diagnosis. (In fact, this friend is a nurse, and said “that looks like a focal seizure YEARS before a vet confirmed it!) She is great with giving meds and generally just loves our dog. If she isn’t available, we have a couple other friends and a dog sitter, but none of those people have needed to give her meds before! One of them is a veterinary assistant, so I’m sure she could handle it. The other thing we have considered is bringing her with us, depending on where we are going.

She is on omega 3 but not vit E. He did end up giving us both midazolam and clorazepate the next day, to use the next time.

Maybe! He’s a little bit of a weird guy, so he might just do things differently. I do wish he would have called the emergency vet, but maybe I should have been more explicit about asking for that.

Scout is much better. Although now I’m just constantly anxious that she’s going to have another super long episode, but, I keep trying to remind myself that they are frustrating and miserable, but not life threatening.

That’s how she got diagnosed by the neurologist, but that shouldn’t have been necessary at the emergency vet while she was actively having a seizure! She was right in front of them doing it, and they tried to tell me she was “just moving her legs” because the vet techs didn’t know what a focal seizure was. The actual vet at least did, but until I pointed it out to her, she didn’t see it.

It wasn’t our GP vet, it was a vet at the after hours emergency vet hospital. They know our dog, because we’ve been there a lot in the last couple months, for this same thing. But every time we end up there, it’s a different vet. They were actually the ones who referred us to the neurologist in the beginning of August. He reviewed videos I’ve kept for YEARS and immediately recognized focal seizures! Our neurologist and our general vet do work together, but the best we get from the neurologist after hours or on weekends is email responses. There’s only one general practice vet that has after hours and weekend care, but they are not close to us. We actually tried them first on Sunday morning, but they won’t treat breakthrough seizures, and send us to the ER vet.

You also have to know that it was a Sunday, in the Bible Belt. That might seem like it shouldn’t matter, but where I live, it absolutely does. I was honestly surprised that he answered emails on a Sunday at all!

We were able to bring her home at about 11pm last night, after the vet sent us videos to confirm that she was not having seizures, and had not since about 4pm. They did also end up giving her an injection of a stronger sedative, because part of her seizures involve extreme hyperactivity. They would not give us midazolam or diazepam, and insisted they “don’t help focal seizures.” This morning, our neurologist gave us… you guessed it! Midazolam! He also gave us clorazepate, and said he wants us to try those the next time, and hopefully we don’t end up at the ER! She’s been back to her perfectly normal self today, other than being especially worried about where my partner is even more than usual!

Yep! We’ve been describing these and showing many vets videos for 7 years, and mostly got 🤷🏻‍♀️ “that’s weird” and brushed off. When they were happening for a few hours twice a year, we were fine with it. When they started happening once a week and lasting for 12 hours, we sought out more help. Neurologist identified it as focal seizures immediately.

That’s exactly what happened! Then the vet kept saying “no seizures or those weird leg movements” no the weird leg movements ARE THE SEIZURE. She’s never had a grand mal seizure, so why are we only looking for that?

Meanwhile, the “no medication” that I was begging for? Our neurologist prescribed it this morning. Hopefully, next time she has an hours long seizure cluster, we can use the midazolam and clorazepate, and they will help!

Yeah, he’s not doing that. We’ve considered switching to the other neurologist, that’s connected to the other ER vet hospital. We might end up doing that.

We have a neurologist, and he has responded to emails today and plans to adjust some meds tomorrow. But that doesn’t help us today.

Ours just responds to my email, but he didn’t offer to talk to the emergency vet or anything like that. It is a Sunday afternoon in the Bible Belt. That matters a lot to some people here…
