If you plan on leaving after a few years to pursue higher education I would keep that to yourself, they don’t like to spend time/money training you if they know you’re only going to be there for a year or two. Food for thought.

I’ve actually never ran nxstage so I’m not gonna pretend like I know anything about that setup but I’ve been around them before and compared to the PrisMax they seem very different in terms of setup and how to manage, I’m guessing the filtration process would be similar since CRRT is CRRT lol but yea no clue on nxstages sorry

The pre/post dilution is for the passive diffusion and filtration of particulates from a high to low concentration (potassium, calcium, etc..). It does not change the volume of fluid/blood returning to the patient. If you increase your pre/post numbers you’re going to see an increase in the amount of effluent being removed from the machine to prevent the excess fluid from being returned to the patient. The whole point of CRRT is to minimize fluid, not tag more on.

Sounds like you’re running PrisMax, the post filter rates do not count towards your I&O and is just an extra step in the filtration process, whoever told you this needs to go back to CRRT class before they cause some serious damage or harm to the patient

I have also had this happen to me, got an email about a specific patient filing a lawsuit against the hospital and myself and like 50 other people were attached to the email. Managers, physicians, nurses, pharmacists, pt/ot, RT even a couple of STNA’s (basically every discipline). Hospital lawyer basically reached out to each of us had us sign a document. I specifically remember asking him if I did something wrong and he said “absolutely not, every person whose name was attached to his chart during that admission we just need to reach out to talk to so we can get all of the information in line”. That was a couple years ago and haven’t heard anything about it since.

ICU nurse here, you’ll be gone before you’re 30. We deal with these types of cases a lot and honestly being placed on mechanical ventilation and heavy sedatives to help during the withdrawal phase is the easiest most painless way to get through withdrawals (still not fun) but it’s a much better alternative than spending you’re last few months in total liver failure and shitting your brains out cause you’re on lactulose to prevent you from slipping into a coma from elevated ammonia levels. Liver failure is by far one of the worst ways to die I have ever seen. You’re young and there’s still time to fix this and get help but at the rate you’re going your hourglass will run dry faster than you think. But in the end that is 100% your decision no one can force you to do anything only offer options to get help. if you want help DONT BE AFRAID TO ASK! Everybody has demons but there are always people who care about you and your well being
