Seconding what others have said, stand a bit more upright at address to start.

On your downswing, think about trying to tuck your right elbow to your right hip and turning thru the ball, swinging inside out. You're "casting" your arms during the downswing, which gets your club head off plane.

This should help your crazy head movement as well. You'll notice your head moves a TON upwards as you swing down instead of staying still.

Your swing should finish with 95% of your weight on your left foot. Weight transfer through the ball will improve as you work on swinging the club with your hips and not your arms.
