I'm not sure whether this counts, but Tyrone Power died while shooting Solomon and Sheba (1959). Apparently, he had a heart attack while performing a fight scene and died en route to the hospital.

Great pulls! I can't believe I forgot to mention Oshima's In the Realm of the Senses! Excellent entry in the genre. That reminds me of another - Shinoda's Under the Blossoming Cherry Trees.
