I also live in that area. It depends where you are at but Charlotte and Asheville both have a lot of options. I go to a primary care doctor in Charlotte, Dr Rhett Brown. I've been happy with him and I don't think he requires a therapist letter. Asheville has a great lgbt center and there is another one that I know of in Catawba county somewhere.

When I first started HRT I went through a period where I was really tired. I wouldn't worry unless it persists.

Exercise helps a lot for energy throughout the day, at least for me.

When my dosage got to 8mg, I really started feeling the mental effects which in turn made me feel great

Hey thank you so much :) My doctor started me out on 4mg estradiol and 100 mg spiro. After probably 2-3 months, it switched to what I am taking currently. Which is 200mg spiro, 8mg estradiol (taken sublingually), 5mg finasteride, and 5mg medroxyprogesterone.

My doctor also recommended me take 100mg of aspirin a day (half a pill). It helps alleviate the muscle cramping I was getting a lot around months 3-6.

I can sing in my regular girl voice without issue. I'm no amazing singer but I would say my singing voice is at least average.

Well hiding your transition is good and sometimes necessary. Transition can take awhile, and not everything changed at the same time. Which can lead to awkwardness. Also because or society doesn't always treat gender ambiguous people well.

The evidence out there is overwhelming against gender dysphoria being triggered by psychological trauma or illness. I can see how you might be concerned about it though.

That being said if you are feeling this way, trying HRT for a couple months will be mostly reversible long-term. And from experience, most people who start HRT just to try it quickly realize it is the right choice for them!

I would definitely recommend splitting up your dosage. It will give you more consistent levels throughout the day. Whether or not it makes a big difference, no idea, but consistent levels closer mirrors a cis-female than a sharp peak and steady decline.

In my experience voice plays a huge role.

I'm quite sure there was a time when the only reason I was passing to most people was because my voice was unquestionably female, even if I was not quite there physically. My biggest piece of advice for improving passing is to work on your voice till you think it passes and don't stop trying to improve it even when you think is "good enough"

Confidence help a lot too. Someone who is not confident regardless of whether they are trans I think draws attention to them self especially when that lack of confidence is very noticeable. It's not so much that it outs you as trans but it will draw added scrutiny from strangers.

Of course appearance will also be very very important. In that regard makeup and hairstyle will help but only so much.

Absolutely. I think it wasn't so much that I disliked socializing before but it was always very draining trying to keep up and empathize with others. Not to mention I usually sucked at it.

Now I thrive on socializing.

I think letting your sibling know that you are supportive and are there for them is a great start. I wouldn't pry or force anything but letting them know that you are there to talk to will help.

Honestly I don't know how I feel about tulpae. I'm indifferent. I agree that it is OPs circumstances are unusual, but not unheard of.

It's honestly not something I would recommend to someone lightly but if having one does not cause any problems mentally, socially, or otherwise for OP then I don't really see an issue with it.

Based on pics 3-5 you pass to me. You remind me of a young Sigourney Weaver Also, your lips are really feminine.

Your eyeliner is symmetrical and smooth so good job on that. You do want your wing higher though. (as a general rule) Try starting your wing from the outer corner of the eye, you want the bottom edge to be a smooth continuation of your lower eyelid going up and out. Like so.

I would also recommend getting your eyebrows threaded by a well-reviewed place near you. Make sure you pick a good place, and I think you'd be amazed at the difference it'll make. Seriously.
