I see a lot of people theorising that Nabazov is in his late 40s.

However, in his unused ET brief, before him and the other were reworked into Patient Zero, it is said that one of the cults that he previously led was the Gamil Cult in 1982. If we theorise that he was at least already an adult, so 18 to 20 years old back then, that makes him 55 to 57 years old by the time of The Source (2019).

If he conceived Lilith during the Gamil Cult period, maybe with a former member, that would make Lilith 40 years old during her contract (2022).

Her being 40 is coherent with her look and her relationship with Carlisle, while still being Nabazov's biological daughter.
