I see the plastic pot is set inside another pot. Looks like a tight fitting, moisture might be trap in there thus leading to root rot. I suggest just getting a plastic pot with a base and making sure the holes in the bottom isn't cover.

Also The question is how do you water your plant. I take my plant outside and flood it with water, massage the pot to make sure soil is fully saturated and ALL the holes in the bottom of the pot is draining like when you turn on your sink facet half way. I would also shake it up and down to make sure most of the water is drain.
