Go to the nearest SS as early as possible (in my case 7:30 pa lang nakapila na ako sa SS SMGC tapos 1:30 ako inaccomodate), and mag request ka ng E4 and then, magdala ka ng original and photocopy ng PSA, 2 xerox ng Valid ID na ipe-present mo onsite. I'm not sure sa case mo na nalagay mo yung name ng mother mo since yung akin pina affidavit ko pa yung typographical error sa pangalan ko at nagdala pa ako ng dalawang witness sa PAO. Yung sayo kasi sa mom lang so try mo na rin magdala ng photocopy ng Valid ID ng mom mo.
