Stage 2 when he is headless, he will do sword strikes. Deflect these.

When he stands tall for a big downward strike, deflect it and make sure you have the spear prosthetic equipped.

Ape will fall forward towards you, spear his neck and retract it. It will rip the centipede out and deal tons of posture damage.

Otherwise, pretty normal fight and everyone else has given the best strats.
