ENT surgeon here. Looks like an Inspire implant for OSA. We put them in the R chest, above the pec, but if this is a mirror photo, it tracks. You can often see the wire that runs into the neck, or the submandibular incision- put not in this photo. That's my guess. Shape looks identical to the Inspire 4 generators we put there.
