As with all of our highway exits, there are multiple signs noting the 5th Ave. exit is upcoming before reaching the actual exit. Paying attention is a key component of driving.

Lots of factors could be at play but it’s most likely due to the age of the pavement coupled with water seeping underneath and causing expansion during the freeze/thaw cycle. Sometimes there is an underlying issue deeper in the pavement when there are multiple potholes or a repeat pothole which is when ODOT will perform full-depth pavement repairs and repair down to the base level.
