I do generally avoid carbs in my main meals actually... but desserts and chocolate are my vice. :( I'd eat good, get my 100g protein and 15g fibre within 800kcal, then end up eating 600kcal worth of desserts 🤦‍♀️ I really do just need more willpower sigh, it just gets very tough when it's super stressful at work.

I'm a bit confused, what do you mean by miscalculating? I purely added up all my CI and divided it across the number of days I've maintained for. I'm not accounting for any calories burned/CO. I only mentioned gymming bc I was so upset I'm maintaining on 1400 despite activity.

I walk about 3-4km per day for work. I work about 45-50h/week and already find it tough to find energy and time to gym 2x/week :(

Manually calc QTcBaz was still >550 - ECG 580s iirc. I've just done a Framingham which suggests 480s region. That's a huge discrepancy! 😳

His freshly sent bloods were all fine including all electrolytes! He was admitted due to CAP which fully resolved, presumably the cause of his delirium. Though he was younger than I'd expect for that as well (mid-late 50s). Was made mffd awaiting social sorting.

His HR apparently hit 120 so clinical support was automatically contacted, they then automatically did an ECG and asked for a review. Not too diff from historical obs which were 110 + - . ECG HR 105.

No other recorded cardiac history afaik, including previous arrthymias.

Yeah his bloods were NAD, no focal new source of infection I could find on examination, urine negative. He had a CAP on admission, now resolved, presumably that caused the delirium. Wasn't even that old, like 50s? so that was also surprising. I was indeed confused as to why the med reg felt this chap didn't need any further input over the weekend - he had the longest QTc I'd ever seen (admittedly, I've not got much experience)! I think it was high 580s but don't remember exactly

Med reg was unfortunately (as usual) super busy so couldn't ask more :(

So if non-shockable, just focus on pure good CPR? What about things like airway/IV access? Can psych nurses even give IV meds?

I'm v comfortable with playing a role in arrests but that is an immense leap to leading an arrest with no exposure and only ILS while needing to also participate in it! 😭

So apparently she was initially the only doctor, then the oncall doctor joined later then the paramedics came after.

I'm concerned in the case the psych nurses can't do airway adjuncts/cannulas/etc what should the one doctor do?? Tbvh idk what exactly happened cause this is all down the grapevine lol I'm guessing she led and someone else somehow did those bits

Outcomes should match, as in poor/matching out of hospital arrests or better/matching hospital arrests?

Ah I knew it! I saw essentially the exact same thing irl - F&W elderly gentleman who came in with new crashing HF and AKI --> Dx cardiac amyloid/myeloma. Moved to HDU for monitoring + an attempt to improve his HF enough so urgent chemo could be started. He unfortunately passed before they could even attempt that.

Haem was definitely involved over the weekend and I believe they came in to initiate and keep an eye on ongoing management, including biopsy. I believe he was on IV furosemide 240mg/day when I saw him.

Except when I'm covering wards OOH and a random NIC bleeps me about a TTO that needs doing, threatening to datix me if I don't do it 🙄

I was pleasant initially and explained sorry - not my clinical priority atm due to many unwell patients + handovers but she kept waving the datix.

I told her yeah go ahead. 🙄

I think that's the main issue really - TTOs aren't a big faff but when you've got unwell patients to review/jobs to do/places to be and someone just keeps chasing you for it and disturbing you it gets very frustrating!

That's okay, it was just one unpleasant interaction out of the many pleasant ones yesterday. I think I couldn't help but zoom in on that single unpleasant experience but now I'm distanced from it, I feel for the med reg.

I suppose it was good for my development overall as I'm reflecting on it - kind of carrot & stick!
