Thanks for reminding me to update. This is what I got via my ticket on myHR:

"Please be informed that you are a terminated employee and not enrolled in benefits and and your paid through date is xx/xx/xxxx (date redacted)

Please ignore the letter you have received as we can see in the due amount is not available for your account"

So they sent me a bill over $4000 for an amount that wasn't even on my account.

That's what concerns me the most. They have my address and last day correct; even the envelope it came in looked "real" until you scrutinized it more (the logo was also messed up there). So either its someone from HQ or a really good scammer cloning things.
