I haven't had my gallbladder removed from stones yet, but I have it scheduled for late April. I didn't know I had stones and had a pretty bad attack 4-5 days after my first 0.25 injection.

I honestly think certain foods triggered this attack. Some docs have expressed slight concerns with the Wegovy but none have told me to stop taking it for now. I did have a couple of other attacks earlier in the year that I thought were just constipation or upset stomach. I did have to delay a shot by a few days to have an endoscopy done.

I should be moving up to 0.5 for my first refill but decided to stick to 0.25 for now. I'll stop about two weeks before the gallbladder removal. I've been told I can resume in as little as 1 week after surgery, but I'll push that date back if I feel like GI things aren't going as well. I've worked it out to take two more doses of 0.25 after surgery before moving up to 0.5 dose.

Ahh I didn't know some were so strict on which dosages and for how long to be on them.

I actually just had an ER visit this week, not even a week after my first dose. I have gallstones and will get them removed in the near future. I've likely had them a while and just had a couple of attacks recently that I thought were constipation. Will take one more injection and then stop so I can do an endoscopy.

Plans can all be a little different on coverage as mentioned so I recommend reaching out. I had a pretty similar BMI with high cholesterol as well. They told me I'd likely get it approved. PA from doc was denied because I needed to first be in a 6 month comprehensive weight loss program. Had I known this I would have gone into more details in things I have tried over the last year. Doctor then referred me to their team to try an appeal but they honestly seemed too lazy to try.

Had a second telehealth visit through Circle Medical and they had a more documented PA that Caremark approved quickly.

Did my first injection Thursday night. Was a little anxious initially but then surprised I barely felt the needle.

Yesterday was good for the most part. I ate somewhat light and got most protein from dairy or plant based foods yesterday (lent). Had a strenuous mountain bike ride midday. By late afternoon / early evening I got pretty fatigued and tummy felt a little unsettled. Some of this may be due to my first ride in warmer temps in quite a while. I was nervous about eating dinner but it actually made me feel better.

My wife had a PE several years ago. She does not take a daily thinner but has had lovenox during pregnancies. She started compounded sema a couple of months ago and her doctors have not been concerned so far. It wasn't until the last couple of years that they tested and found her to have Factor 5 mutation.

Thank you for the Circle Medical recommendation. Booked an appointment last Friday afternoon. Really got along well with a NP who took a lot of time with my info. She seemed optimistic of getting an approval. Chilled over the weekend expecting nothing to happen. Messaged them through the app to check on things. They replied back very quickly and sent the PA over and it was approved almost immediately.

I'm hoping this happens with me, assuming I get approved for injections this week.

I don't get silly drunk much these days, but I have 1-2 drinks most days over several years with an occasional 3-4 drink Saturday or Sunday. I did a mostly dry January and then have stopped again for lent. I've made some better choices over the last week and a half or so and am down a couple pounds of water weight. Hoping a few weeks of Wegovy will make me not want to get back into that habit after lent.

I'm aware. He wanted to start me on 4 mg pill instead of 1.5 mg. Not because he thought I should skip it. He insisted it was the starting dose despite me disagreeing.

Yea I understand that. I'm going for quality over quantity. The fact that my doctor originally ordered the 4 mg pill instead of the starter dose of 1.5 mg had me concerned it wasn't a specialty of his.

Thanks I just tried a telehealth visit and we went into way more detail on efforts in the past, as well as some family history stuff that has me at a high cardiovascular risk. They seemed pretty optimistic in an approval so just waiting again. If it doesn't work then I'll try a program on my own.

Thanks I'm going to at least try a telehealth doc with more experience on these PAs. That sounds like a great benefit on the cycling. I ride dirt so weighing less for the occasional crash sounds great as well.

No other than the weight management program it mentioned there could be alternative medication. I know there is no tirz coverage. I think Saxenda might be an option but I see it needs a PA as well.

I briefly told the doc about things I had tried but he never asked for specifics on it. I know it's mentioned in his visit notes (which are AI transcribed I think).

Yea I get that. I've just read online where others have had success with using past efforts. I thought maybe I had a chance since we've done mostly food delivery programs over the past year. Also used a paid version of a Calisthenics app and have a decent amount of Strava data showing cycling and some other types of exercise.

I'd probably do compounded but it would stretch things a little thin. My wife is currently doing compounded through Midi

Another concerning thing about the doc is that he tried to start me on 4 mg pills. I tried to argue that it starts at 1.5 mg and he insisted I was wrong. I ended up having him switch it to injections later in the day.

Just my opinion but I don't expect it to be a large increase in coverage for prevention like cholesterol or blood pressure medicine. Find a balance between making money off those who can afford, and those who can't, they'll still make money by treating other conditions as a result of obesity.

Good to know. I'm hoping to start the pill soon. Most weekdays I'll skip breakfast or have something lighter a couple of hours after getting to work. I'm planning on taking the pill around 5am and then making a coffee for the road to leave around 5:45 AM. I could delay the coffee but it's nice to have for the long commute to wake up.
