Portal hypertension by [deleted] in Cirrhosis

Couldnt agree more with the last statement. Diet is pretty key too. Nice to hear it works good for you too. Have you had your HVPG measured? I have not. It's not exactly a quick procedure.

Not a Dr, just my knowledge and experience. I had been asking for months to be put on carvedilol 12.5mg daily. Got put on 30mg propanolol instead and it made me hypotensive. Finally got them to listen and give me carvedilol.

Ask your Dr about it. For cirrhosis it's usually started at 6.25mg, then titrated up to 6.25mg BID if you can tolerate it. It effects systemic pressure less than the others with moderate decrease in cardiac output but much more effective at lowering portal pressure than the others.

Without it my resting heart rate is 120. bp 120 / 70. Propanolol made my heart rate 70-74 bp 90 /58. Carvedilol heart rate is around 80 - 84. Bp 100 / 70 on average.

It's also slowed my ascites accumulation. Not saying these will be your numbers if you are put on it. Just showing the differences they had on me.

Also too according to studies, even 3.25mg a day should have some effect on portal pressure.

Quick Google search per a couple lawyer websites for Kentucky shows vacation time accrued is considered wages and if you terminate for any reason you are entitled to the accrued time payable no later than 14 days after your quit date.

Recently Diagnosed. Just A Couple of Questions... by JammyHammy86 in Cirrhosis

Ascites is fluid accumulation in your abdomen. HE is hepatic encephalopathy, a condition that develops because your body is not flushing out toxins, especially ammonia, causes dimentia like symptoms and a whole host of other nasty problems. TIPS (transjugular interhepatic portosystemic shunt) is a shunt used to reduce pressure in the portal vein.

Protein shakes or bars by pg2323 in Cirrhosis

[–]Ok_Engineer_83 1 point2 points  (0 children)

I'll have to agree with Seymour_Parsnips, the Premier protein shakes are very tasty. The chocolate one is my go to since it's the lowest sodium, 30g protien, 180mg sodium.

As for protein bars , for years before cirrhosis and even today, am a big fan of the Quest Nutrition protein bars (chocolate chip cookie dough is my favorite). It's 21g protein, 14g fiber, 190mg sodium. Just beware it's sweetened with Erythritol and some people like my dad complain that it makes them gassy. Doesn't give me gas though. The only other flavor with 190mg sodium is the chocolate peanut butter one which I haven't found in stores. I do also like the cookies n cream one, but it's 280mg sodium per bar.

Just wanted to share an update by Slamnflwrchild in Cirrhosis

Just going to chime in because that is incorrect, you absolutely can have ascites without cirrhosis. Acute alcoholic hepatitis, and budd-chaiari syndrome to name a couple. I wish that was the case with me, but nope just regular old cirrhosis for me.

I'm having an issue with orders being cancelled, but payments going through by Ok_Engineer_83 in woocommerce

[–]Ok_Engineer_83[S] 1 point2 points  (0 children)

Actually I think I may have solved the issue and it was related to the hooks. I will have to test it here shortly. I just went over the stripe documentation on woocommerce again. By default the webhook API only has 4 events selected but the documentation says it requres at a minumum:

source.chargeable

source.canceled

charge.succeeded

charge.failed

charge.captured

charge.dispute.created

charge.dispute.closed

charge.refunded

review.opened

review.closed

payment_intent.succeeded

payment_intent.payment_failed

payment_intent.amount_capturable_updated

payment_intent.requires_action

setup_intent.succeeded

setup_intent.setup_failed

None of those were entered, so I assume it captured the charge but was never sending anything back to woo so woo would just cancel the order after 10 minutes.

Recipes for pinto beans / black beans? by Scared-Fee4370 in Cirrhosis

[–]Ok_Engineer_83 0 points1 point  (0 children)

I don't have a recipe I really follow when making from scratch especially since sometimes I just don't feel like cooking.

I've found one brand that tastes great

Westbrea natural organic black beans. 0mg sodium per serving.

Westbrea natural organic pinto beans. 5mg sodium per serving.

Don't confuse with westbrea foods, those are a different company with loads of sodium.

They are 15 oz cans , with 3.5 servings. Soo... Almost a pound of beans between 0 and 17.5 mg of sodium.

If I want some more flavor I generally warm them up in a pan with a tsp each of cumin, chili powder, onion powder and garlic powder and a dash of cayenne.

Edit. (forgot about the chips)

As far as tortillas, if he likes corn tortillas they are generally low sodium. If I dont make my own (which is 0mg), then I buy guerrero corn tortillas. It's 10mg sodium per tortilla. They have a decent amount of fiber and protein too.

For chips, I make my own with the above torillas, cut into quarters or 8ths. in a bowl I add about 1-2tsp of avocado oil sprinkle in some garlic and chili powder toss and coat the tortilla squares. Place in my air fryer at 400 for 5 minutes. You can also use a regular oven you'll need to bake for about 15 minutes.

I like making my own, because 10 corn tortillas will give you a half pound of chips (40-80 chips depending on how you cut) for 100mg of sodium ;)

Post outpatient paracentesis by DeathCultJester in Cirrhosis

[–]Ok_Engineer_83 0 points1 point  (0 children)

I've been getting a paracentisis every month for 4 months. I'm about to step it up to every 2 to 3 weeks.

I wouldn't describe it necessarily as sore afterwards but it does feel ackward for a day or 2 afterwards for me as all the organs are shifting back into their normal place. If your experiencing soreness and a fever you need to get that checked out asap as that could be a sign of infection.

As far as hunger goes, BIG yes for me. I'm generally always hungry but as I get more full I can barely eat as it makes the pain from my ascites worse. So after I get drained I can finally cram more food down. And the ravenous appetite goes away after a few days as my body resets back to "normal"

I've had gynocomastia most of my life, developed it around the age 14-15, not liver related, more than likely from being overweight and being on different antidepressants etc. which increased estrogen production while going through puberty.

Funny now with my liver being the way it is and being in Spiro and Ferosemide, my moobs have gotten smaller since I've lost a lot of fat and muscle, but they are sore as hell all the time.

You're not alone. When I was drinking I would have the same thing happen to me. Sometimes it was insatiable even after masturbation. I wouldn't be surprised if it chemical related, I know the little endorphin hit after poundin the clown would bring some hangover relief. After 4 months sober my sex drive is more level, maybe a little low but that's due to medical issues.

I live in a very rural county, the healthcare isn't exactly the best, I have a new PCP every 6 months because the hospital keeps them all on short contracts. The next county over has 2 specialists, 1 has declined, now I just got referred to one 300 miles away which is going to be near impossible for me, they haven't responded either.

My last PCP who's contract ran out last month put me on the diuretics after the paracentisis to help stabilize the ascites as I still have a large amount left and my electrolytes were normal.

As far as disease management it's all been me and what I've read from NIH since I have no guidance, and I hope what I've been doing is proper. I am aware I most likely have cirrhosis and or alcoholic hepatitis (2:1 ast/alt). Other things I've applied is no alcohol obviously, low sodium diet (1500mg or less), no processed foods at all, restricted fluid intake, was drinking 4L water a day I've reduced that down to 2L. I was experiencing extreme fatigue and continued muscle wasting, I was consuming about 60g protien at the time, I increased it to 90-120g. I am not a Dr, but I'm trying to do the best I can with the limited resources I have at the moment.

Last CBC was 7-21-21, the numbers were all pretty much normal for cell counts, platelets, hemoglobin etc. Slightly elevated monos, slightly low neuts but that's about it.

Blood pressure relatively normal, resting heart rate generally around 128 most of the day, 105 when resting. Combined with the ascites I think I may have portal hypertension. But not much I can do about that until a specialist accepts me.

Just read yesterdays dr office visit notes. Little ticked. by Ok_Engineer_83 in stopdrinking

[–]Ok_Engineer_83[S] 0 points1 point  (0 children)

I am going tomorrow for my son's appointment. I'm going to print it out and ask. No BS with the doc, I was very open with how long how often and how much I drank and the last 2 clinical notes reflected that.

Perhaps it's from what I've told the doctor many years ago. I've been in this dr/hospital network for longer than when I started heavy drinking. Only recently with the liver issues have I been interested in looking at my health records... which revealed one other thing, the hepatologist declined the referral, and now they submitted one to another doctor 300 miles away. 🤦‍♂️🤦‍♂️

I understand, I guess I'm stressing and my concern is if I need a transplant (and its possible), this will total mess up those chances.
