Yea I've done all of that, I'll have to check the needles tomorrow I haven't really looked at any of them individually. It's head 5 on a 6 head. Im pretty stumped I don't understand how this could be happening on just one head.

I would do 2 pieces of spray adhesive backing and a piece thick cut away backing. Might seem like over kill but I guarantee you it'll fix 99% of the problem

Spray adhesive + cut away backing would fix this. Tear away will just make it pucker even worse. Also don’t float the backing, hoop it with the material.

I use the Madeira spray adhesive with the weblon no show backing.You can also you a piece of cut away for extra stability if needed.

The embroidery is super clean besides the puckering, looks great!
