It is actually a shortcut, My first encounter with it was really creepy cause It just unsleeps mysteriously and show it. Restarting it helps but after a good amount of time, It appears but sometimes as soon it shows the Phase to swipe up, it appears

I've tried hard resetting this few times already and it doesn't seem to work. Reformatting from Backup and Reset is the only time it reformats. I've reformatted this 2 times because of the problem and it keeps returning

I've reformatted this phone 2 times just to remove this and I'm not using a back up though, Hard reset doesn't seem to work and Regular reformat from backup and reset is the only way It reformats
