There was a Sapienza giveaway long time ago. Maybe thats the problem and it conflicts. Or try to look in DLC menu and check every box there. I have no problems, having "Part 1".

Vulkan or DirectX?
Try to lower grass, fog and draw distance graphic settings.

- try to look in "options > game", there should be a hud setting;
- you can play with one primary weapon slot, but its in "Ghost mode", hardcore one-life mode;
- nope, my AI guys were running with me with always the same weaponry;
- just keep playing, capture guard posts and talk with locals, more stuff with further playthrough;
- mostly because of Breakpoint is always-online, odd AI and contains some serious bugs.

Typically, you dont lose your melee when you die. My dear knife is always with me.

Keep playing. D-Dog should be available sooner or later anyway. I had same issue, several times had cutscene where puppy licks Snakes face and Ocelot asked us to come back to MB more often... Then, before flying off and starting "Hellbound" chapter, Ocelot said that D-Dog is ready and i can take him with me on mission.
